How we built this table

This table covers GLP-1 medications prescribed for weight loss under fee-for-service Medicaid and each state’s own employee health plan. Diabetes coverage is a different question: most programs that exclude weight loss still cover GLP-1s for diabetes and other approved uses. Local-government, university and teacher-only plans are not included.

The Medicaid baseline is KFF’s January 2026 analysis of state Medicaid coverage of GLP-1s. Every state that covered GLP-1s for weight loss, or changed its coverage recently, was then checked against its own Medicaid agency or legislature. “Yes” means covered with prior authorization at roughly the label’s criteria; “limited” means stricter criteria (for example a higher BMI) or coverage with a stated end date. State employee plans were checked on official plan pages, or official reports quoting the plan; where that could not be done, the row says “not verified”.

GLP-1 medications approved for chronic weight management include Wegovy® and Zepbound®¹. Coverage rules name the drugs a program pays for; ask your plan which ones, and what it requires, before you start.
