The atom at the very center of the corrin ring. Coordinated by four pyrrole-derived nitrogens in the plane of the macrocycle, plus one axial ligand above and one below.

Cobalt is the catalytic atom B12 uses to shuttle methyl groups. The enzyme methionine synthase requires a cobalamin cofactor to convert homocysteine to methionine — one of the body’s primary methylation reactions, required for DNA synthesis and red blood cell formation.

B12 is essential for hematopoiesis, neurological function, and one-carbon metabolism. Cobalamin deficiency is associated in published research with macrocytic anemia, peripheral neuropathy, and elevated plasma homocysteine.

Compounding pharmacies are licensed by their state board of pharmacy and operate under the FDA's 503A (patient-specific compounding) or 503B (outsourcing facility) framework. This is the same legal pathway that has been used for decades to prepare custom dosages, allergen-free formulations, and patient-specific prescriptions.

A generic drug is FDA-approved as bioequivalent to a branded product. A compounded preparation is not. The preparations REMEVi prescribes are not FDA-approved finished drug products, are not generic versions of any FDA-approved drug, and have not been clinically studied as finished products.
