Healthcare services arranged through REMEVi Telehealth are delivered by licensed providers contracted with or employed by OpenLoop Healthcare Partners, PC and its affiliated state professional corporations. Before receiving services through REMEVi, you must review and consent to the OpenLoop Healthcare Partners, PC Telehealth Consent Form.

To ensure you always see the most current version, this consent is maintained directly by OpenLoop Healthcare Partners, PC. Any updates they publish take effect immediately for all REMEVi patients.

What You Are Consenting To

The full consent form covers, among other things:

Use of videoconferencing, telephonic, and asynchronous technology in lieu of in-person care
Provider review and access to your medical records and prescription history
Privacy expectations during your visit
Risks and limitations of telehealth (interruptions, delays, data processing errors)
Use of artificial intelligence (AI) tools, including transcription, clinical decision support, and ambient listening, with the right to ask questions or opt out where feasible
Possible recording of the visit for quality assurance and care coordination
Acknowledgement that your provider may be a licensed provider, nurse practitioner
Authorization for OpenLoop and the Practice to use and disclose your medical information for treatment, payment, and operations
Consent to receive text messages and emails related to your care
Assignment of insurance benefits, where applicable

Emergencies

Telehealth is not appropriate for medical emergencies. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
