Then in 2024, Eli Lilly published the SURMOUNT-5 head-to-head trial — directly comparing tirzepatide vs. semaglutide for weight loss. The result: tirzepatide produced significantly greater weight loss than semaglutide at 72 weeks.

Bottom line: On average, tirzepatide produces about 5–7 percentage points more weight loss than semaglutide. That difference matters more for patients targeting larger weight reductions.

That said — semaglutide is still highly effective, costs less, and may be the right choice if you respond well or have a smaller goal. The “best” GLP-1 is the one you can stay on consistently.
